We would like to thank you for taking part in the process of accepting athletic participation paperwork online. This automated process will allow us to be more efficient in handling the forms as well as saving valuable resources.
In the upper right-hand corner, click Fill Out Electronic Forms. Then, select Texas under the State dropdown menu. Scroll down and click on Cedar Hill. You will be directed to the Cedar Hill Rank One Sports Page. You can begin filling out information or you can click on REGISTER NOW and create an account. Having a registered account allows you to update any information throughout the school year.
To access the online forms hold your cursor over the “Electronic Participation Forms’ tab. You will get a drop down list of the forms. To sign the document, click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over, click on the refresh icon next to the signature box. If you are using an IPad, you may create an electronic signature by holding your finger to the screen and signing your name or by using a stylus.
Once you have filled out all of the information on each page you will have the opportunity to print the document.
You will receive a confirmation email one the document has been reviewed by the CHISD staff.
The hard copy of your Pre-participation Physical Examination Form must be turned in to your student’s campus athletic trainer. All other forms will be submitted electronically.
This paperwork including your child’s physical, medical history, any additional medication forms (if applicable) and athletic fee must be turned in no later than 24 hours prior to the first day of practice or school whichever comes first. There is a 24 hour approval process once all documents are completed.
The Cedar Hill Athletic fee is for each student participation in our athletic program. This fee helps offset the cost of laundering player’s uniforms, workout clothes, towel, etc. A student may participate in more than one sport at no additional fee. This year's athletic fee will be $25.00 for each high school student and $50.00* for each middle school student participating in one or more athletic activities. Students with special financial circumstances should discuss payment options with their coach at the start of the school year. You can be assured of confidentiality regarding payment arrangements. Every effort will be made to ensure that all students who wish to participate in athletics are given the opportunity. You can pay cash or money order made payable to CHISD - Athletics or pay your athletic fee online @ MyPaymentsPlus.
* Middle School athletic fees includes a set of practice clothes (t-shirt and shorts)
